Early Hepatic Encephalopathy after a Transjugular Intrahepatic Portosystemic Shunt Procedure: Case Report

چکیده
Patients with decompensated hepatic cirrhosis may present systemic alterations and dysfunction of multiple organs. Ascites, portal hypertension, esophageal varices, encephalopathy are common complications arising from cirrhosis. The aim this paper is to report a case patient liver the transjugular intrahepatic portosystemic shunt. Male, elderly, ex-alcoholic, diagnosed cirrhosis, ascites, varices. He underwent shunt due hypertension returned hospital after 24 hours agitation mental confusion. had bowel movement stop, neurological worsening, loss renal function, hydrothorax, encephalopathy, hypernatremia, hypocalcemia, hypophosphatemia. new procedure occlude shunt, showing improvement status ascites. However, continued decompensation hydro-electrolytic disorders. evolved worsening ventilatory pattern, fatal outcome. Esophageal varices can be corrected such as occur. Therefore, there need for reintervention or reduce its caliber. Thus, patients advanced age potential risks benefits should carefully evaluated risk death.

Severe Hepatic Encephalopathy after Transjugular Intrahepatic Portosystemic Shunt (TIPS): Value of Shunt Reduction and Occlusion
Hepatic encephalopathy (HE) after implantation of the transjugular intrahepatic portosystemic Shunt (TIPS) is generally well treatable. Severe and refractory HE might be treated with shunt reduction or occlusion. We performed a retrospective study between January 2004 and January 2016. Out of 456 TIPS implantations, 17 patients (3,7%) were treated with shunt reduction and 6/17 patients with add...
متن کاملPathological Predictors of Shunt Stenosis and Hepatic Encephalopathy after Transjugular Intrahepatic Portosystemic Shunt
Background. Transjugular intrahepatic portosystemic shunt (TIPS) is an artificial channel from the portal vein to the hepatic vein or vena cava for controlling portal vein hypertension. The major drawbacks of TIPS are shunt stenosis and hepatic encephalopathy (HE); previous studies showed that post-TIPS shunt stenosis and HE might be correlated with the pathological features of the liver tissue...

The transjugular intrahepatic portosystemic shunt (TIPS) is an interventional treatment resulting in decompression of the portal system by creation of a side-to-side portosystemic anastomosis. Since its introduction 16 years ago, more than 1,000 publications have appeared demonstrating broad acceptance and increasing clinical use. This review summarizes our present knowledge about technical asp...

BACKGROUND Since its introduction, transjugular intrahepatic portosystemic shunt (TIPS) has been extensively used for treatment of portal hypertension. We report a decade of experience with particular emphasis on characterizing post-TIPS hepatic encephalopathy (HE). AIM To determine the frequency of clinically evident or minimal HE post-TIPS, identify predisposing factors and determine the im...

Purpose. The purpose of this study was to determine the incidence and predictors of hepatic encephalopathy (HE) after transjugular intrahepatic portosystemic shunt (TIPS) and endoscopic therapy (ET) in the elective treatment of recurrent variceal hemorrhage. Methods. Seventy patients were treated with elective TIPS and fifty-six patients with ET. Median observation time was 46.28 months in the ...
